Program Description

A targeted case manager works one-on-one with the child and guardian at home,school and in the community to meet the client's needs which can include mental health and medical assistance, family support/education, educational support, financial advocacy and linkage, legal referrals and advocacy and social needs.

Must meet certain Medicaid eligibility requirements to participate in case management services. Accept Medicaid and United Health Care, Healthease, Sunshine State, Coventry (vista)*** Required Documentation: Birth Certificate, Social Security card; If legal guardian is not the parent, custody papers signed by a Judge.
