Program Description

The role of the Targeted Case Manager is to link recipients with the appropriate community resources in order to empower our children and families to become self efficient. * Provide Community Resources for children & their families * Prevention of out of home placement * Advocacy/Linkages to appropriate mental health services

Reside in Circuit 19; Children 5-18 years of age, family involvement; medicaid recipient*** Birth certificates; driver's license or other photo ID; social security card; proof of residency or citizenship; proof of income/W2.
