Eligibility Requirements and Important Information Youth entering therapy are frequently involved in more than one system of care. They may have an assigned Child Protective Services (CPS) worker as well as a Probation Officer. They may be in special classes in school, or attending an alternative school. They may have been placed in a group home or foster care at a young age, or may still be living in an out-of-home placement*** Social security number, date of birth, medicaid number, financial information required. |
