Eligibility Requirements and Important Information

Eligibility Criteria: Ages 11 ? 21 years old Has a mental health diagnosis or co-occurring mental health and substance abuse diagnosis AND Has one or more of the following accompanying characteristics: ~Is at risk for out of home placement as demonstrated by repeated failures at less intensive levels of care ~Has had two or more periods of hospitalization or repeated failures ~Has had involvement with the Department of Juvenile Justice or multiple episodes involving law enforcement ~Has poor academic performance or suspensions
