Program Description

Provides information, guidance and counseling, training, adaptations, job placement and related supportive services. Transition services are provided for youth beginning at age 14 through their educational years. Youth are encouraged to try various work experiences to prepare them for eventual job placements in careers of their choice.

The Application for Services form is available online or for download in both English and Spanish. A Doctor Referral Form (sometimes called an Eye Medical Report) is also available for download by Doctors referring a client to DBS.
