Program Description

The Community Action Team (CAT) is a multi-disciplinary team consisting of mental health counselors, case managers, therapeutic mentors, a nurse, and a psychiatrist who provide comprehensive, intensive, community-based treatment to families with children at risk of out-of-home placement due to a mental health disorder. The CAT Team provides family-centered, culturally competent services focused around the strengths and needs of each child and his or her family with a goal of supporting and sustaining the child in his or her family system and community. Medical staff provides psychiatric care, basic health status checks and works with the family to maintain medical records and linkages to community health care practitioners. The CAT Team assists the family in developing a natural support network, improve interactions with the school system, and develop and use other community resources and supports.

Eligibility Requirements and Important Information

Eligible clients are children who have a mental health diagnosis or co-occurring substance abuse diagnosis. To qualify for these services children ages 10 and under must meet two of the following criteria, children 11-21 must meet one of the following criteria: the youth must be at risk for out-of-home placement as demonstrated by repeated failures at less intensive levels of care, 2 or more hospitalizations, involvement with Department of Juvenile Justice or multiple episodes involving law enforcement, or poor academic performance and/or suspension.
