Program Description

Your Plate Health & Wellness Center is an innovative approach to alleviating poverty and food insecurity at one location. This program consists of SNAP, FL Kidcare, Medicaid, TANF, and other benefit assistance; nutrition education; health & wellness workshops; economic mobility presentations; senior programs; child nutrition programs; and more . Your Plate Health & Wellness provides individuals and families with the tools they need to achieve independence, health, and financial stability. We speak English, Spanish, and Creole.
